Employee questionnaire: sample survey you can use

Ever heard of Twenty Questions? It’s a guessing game, and it’s supposed to be fun. But it’s no fun for employers if they have to guess whether employees are happy on the job.

The workplace version of this game is made even worse because employees are often reluctant to complain about what might be wrong with their company, their team, or their individual job. That’s where an employee questionnaire can help. It gives workers a chance at real, honest communication with their managers. That two-way communication is the biggest motivator for retaining employees.

To help you better understand your employees’ attitudes, we’ve created a form below as a guide to create your own employee satisfaction survey. It’s an anonymous questionnaire — with less than 20 questions.

Sample Employee Survey

Please take a few minutes to complete this employee questionnaire. Since the goal of this research is to continuously improve our processes, please be constructive and honest in your feedback.

Collect the data and act accordingly

You can print out the form or copy and paste it into an electronic document for email distribution. Either way, you have to follow up to make the employee satisfaction survey useful.

Collect the responses to the employee questionnaire, and then do your best to make necessary changes. If you don’t address the problems, you run a higher risk of employee turnover.
